What is Cremation?

Although cremation dates back to ancient times, it has only recently become a more common choice throughout the United States. People choose cremation for a variety of reasons. Those reasons may be based on environmental considerations, economic factors, or philosophical or religious beliefs.

Many people believe that choosing cremation limits the options of paying tribute to loved ones. Actually, there are a variety of options available with cremation. Most families hold services, which help the bereaved cope with the loss of a loved one. Many people find that opting for cremation gives them the opportunity to create and personalize the various service options for a more meaningful experience.

The greatest misunderstanding about cremation is the belief that memorial services are not needed. However, a service or memorial ceremony is an important step in helping the bereaved overcome their grief, and family and friends the opportunity to honor a loved one.

Services or ceremonies can precede or follow the actual cremation. Prior to cremation, there may be a gathering, which can be either public or private, with an open or closed casket.

When the service follows the cremation, a receptacle (or urn) containing the cremated remains may take a place of prominence. Ceremonies can be simple, contemporary, religious or secular. Music, readings, stories and poetry can be added to personalize the event to both reflect on and celebrate the life that was lived.

Cremation Authorization Form


Fill in the information at the top of page 1. Read the entire first page and initial at the bottom of page 1 that you have read the information listed on page 1. If the Cremation Remains are to be shipped via US Registered Mail, list the name of the individual to receive the Cremation Remains and their mailing address. Make sure to initial next to this information.

If the Cremation Remains will be picked up, list the name and relationship of the individual that is allowed to receive the Cremation Remains. Make sure to initial next to this information.

If personal property is to be removed by the funeral home, please list the property in the appropriate area.

The legal individual(s) authorizing cremation must agree to cremation and fill out the next area of information requested accompanied by a signature.

Death Certificate Information


Fill out all information on the requested form. If information is unknown, list unknown in the blank.

Disclosure/Disclaimer Statement


Call the Funeral Home and request that a General Price List be emailed to you. Once you receive the General Price List, sign in the appropriate area acknowledging that you received a copy. The Funeral Director will also email a Statement of Funeral Goods and services selected (The purchase Contract).

Embalming Authorization/Refusal for Embalming


If the deceased is to be embalmed (for viewing purposes), sign the Authorization to Embalm. If the deceased will not be embalmed (no viewing), sign at the bottom of the page stating "Refusal to give Permission to Embalm".

Credit Card Charge Authorization


If you choose to pay by credit card, fill out the information on the requested form.

Once all forms have been completed, fax the forms to 409.986.9903, scan the forms with all information and email to Funeral Director,or make other arrangements with the funeral director.
